Mail Order Application Form
This form is required for all purchases of firearms, magazines, firearms parts and ammunition when delivered by courier, freight or the Dealer Network. It will need to be completed and forwarded by you to FSA. We will provide additional information such as serial numbers to you via email. Once we receive the processed and approved form from FSA we will dispatch your order.
This form is not required for pickup or delivery where we can personally sight and verify your firearms license.
Application for Permit to Possess
This form is required for purchases of Pistols, Prohibited Firearms, Restricted Firearms, Prohibited Magazines and Parts for the above firearms. It will need to be completed and forwarded by you to FSA. We will provide additional information such as serial numbers to you via email. Once we receive the Permit to Possess from FSA we will dispatch your order along with the paperwork.
